Head-gasket failure
The 1FZ-FE is well known for head-gasket failure, most likely on high-km engines or any that have been overheated. Coolant loss, overheating and combustion gases in the coolant are the signs.
Fix: Replace the head gasket (machine the head if needed) and fix the underlying cooling problem; verify the head is straight.
