Ceramic turbine wheel failure
The factory T28 turbos use ceramic exhaust wheels for fast spool, but they shatter under detonation or boost much above ~1.0 bar, sending debris and leaving a turbo (or two) dead. Many tuned cars have already grenaded the originals.
Fix: Replace with steel-wheel hi-flow or aftermarket turbos; never run boost on ceramics, and ensure tuning/fuel quality prevents knock.

Oil pump drive collar failure (pre-May 1993)
Early RB26 (and RB25/RB20) use a narrow oil-pump drive that only minimally engages the crank-snout flats. With the 8000rpm redline and a mis-shift over-rev, the drive can crack the pump gears, oil pressure collapses and the engine spins a bearing. Nissan didn't widen the crank snout until May 1993, so the AU-delivered 1991 cars are the vulnerable design.
Fix: Fit a billet crank collar and an upgraded (N1 or aftermarket) oil pump, and monitor oil pressure. Treat this as mandatory before any spirited driving.

Oil pump drive collar failure (engine-destroying)
The RB26 oil pump is driven off a collar on the crank snout; the early narrow-engagement design can lose drive when the engine is over-revved, shock-loaded or run at sustained high rpm, dropping oil pressure and spinning bearings. R33 cranks are generally better than R32 (some have a wider collar) but it remains the defining RB26 failure - and many imports have been thrashed.
Fix: Fit an aftermarket crank collar and upgraded (N1-style) oil pump during any rebuild, run a quality oil-pressure gauge, and avoid over-revving. Confirm whether the collar mod has been done.

Tuner/abuse and authenticity risk
High-value, heavily-modified, sometimes crash-repaired or GTS-t-converted cars. A blown or tired RB26 is a five-figure problem, and fake GT-Rs exist.
Fix: Independent specialist inspection, compression/leakdown and boost-leak test, chassis/VIN verification, full import auction-sheet and PPSR check before buying.

Timing belt failure
Same belt service - failure = bent valves on legendary motor.
Fix: Belt + tensioner + water pump every 100k. Use OEM or Tomei parts only.

Oil pump drive failure (catastrophic)
OEM oil pump drive collar known weak point - shears under hard use. Engine destruction follows.
Fix: MANDATORY billet oil pump drive on any modified RB26 (Tomei, Nitto). Many owners do this preventively.

Gearbox synchro and clutch wear
The 5-speed manual wears 2nd/3rd synchros and the clutch hard, especially on launched/tracked cars. Crunchy downshifts are common on tired examples.
Fix: Synchro/gearbox rebuild and a quality clutch; inspect for AWD driveline play during the test drive.

OEM twin ceramic turbines shatter under increased boost.
Fix: Aftermarket twin steel/billet turbos or single-turbo conversion. Mandatory if power is increased.

HICAS rear steering fault
Hydraulic rear-wheel steering develops faults over time. Many owners eliminate the system entirely.
Fix: HICAS lockout kit (~$200) removes the system. Or full rebuild.
